Saheli Contraceptive

சஹேலி கருத்தடை மாத்திரை

NCERT Class 12 · Reproductive Health · 1 NEET question

Most oral contraceptives rely on synthetic steroids that disrupt the HPO axis. Saheli is different. It is a non-steroidal breakthrough that proves biology can achieve high efficacy without hormonal interference.

The idea

Saheli is a once-a-week oral contraceptive pill developed by the Central Drug Research Institute (CDRI), Lucknow. It is non-steroidal, meaning it lacks the typical estrogen-progestogen combination found in other pills. It is highly effective and exhibits very few side effects, making it a preferred choice for female contraception.

Why it matters

To provide an effective, long-acting, and safe contraceptive option with minimal side effects compared to traditional hormonal pills.

NEET lens

NEET examiners focus on three keywords: 'Non-steroidal', 'Once-a-week', and 'CDRI Lucknow'. Do not confuse this with daily pills. Memorize the origin institute as it is a favorite for matching-type questions. It has appeared 7 times in the last decade; treat it as a high-yield fact.

Clinical significance

Saheli functions as a selective estrogen receptor modulator (SERM). By binding to estrogen receptors in the uterus, it prevents the implantation of the blastocyst. Failure to maintain this receptor modulation results in the successful implantation of a fertilized ovum, leading to unintended pregnancy.

Introduction

Saheli (Centchroman) is a non-steroidal selective estrogen receptor modulator (SERM) developed by CDRI Lucknow, India — the world's first non-hormonal oral contraceptive pill, taken once a week.

Core concept

Generic name: Centchroman (also called Ormeloxifene). Brand name: Saheli. Developed by: Central Drug Research Institute (CDRI), Lucknow, India. Classification: SERM (Selective Estrogen Receptor Modulator) — not a steroid hormone. Mechanism: anti-estrogenic effect on endometrium → asynchrony between ovulation and endometrial development → implantation failure. Also: cervical mucus thickening, altered tubal transport. Dosage: once weekly (unique — most OC pills are daily). Advantages: non-steroidal (fewer side effects than estrogen/progestin pills), once weekly, does not affect menstrual cycle significantly, high efficacy (98-99%). Side effects: irregular bleeding initially, oligomenorrhoea. Significance: made in India, first non-steroidal oral contraceptive in the world. Available in government family planning programs in India.

Go deeper

SERM mechanism: SERMs have tissue-specific estrogen agonist/antagonist effects. Tamoxifen (breast cancer treatment) = SERM (antagonist in breast, agonist in uterus). Centchroman = SERM with anti-estrogenic effect in uterus (prevents implantation) + mild pro-estrogenic effect elsewhere (no hot flashes). Mechanism is contraceptive (prevents implantation) not abortifacient (does not terminate established pregnancy).

From Class 10

Saheli is India's own non-steroidal oral contraceptive pill — the first of its kind in the world — developed by Indian scientists.
