Surgical Sterilization

Introduction

Surgical sterilization consists of permanent contraceptive procedures: vasectomy (vas deferens ligation/occlusion in males) and tubectomy/tubal ligation (fallopian tube ligation/occlusion in females), the most effective (>99.9%) but largely irreversible contraception.

Core concept

Vasectomy (male): vas deferens cut/tied/cauterized under local anesthesia (outpatient, 15-20 min). Sperm no longer in ejaculate (sperm reabsorbed). Testosterone and sex drive unaffected. Effectiveness: >99.9%. Post-procedure: not immediately effective (residual sperm in tract — use backup contraception for 3 months or until semen analysis shows azoospermia). Tubectomy/Tubal ligation (female): fallopian tubes cut, tied, banded (rings/clips), cauterized, or occluded (Essure - now discontinued). Done via laparoscopy or mini-laparotomy. Local/general anesthesia. Effectiveness: >99.9%. No effect on hormones/menstruation/libido. Reversal: vasectomy reversal (vasovasostomy) possible — 40-90% success. Tubal reversal: lower success rates, IVF often preferred.

Go deeper

Vasectomy vs tubectomy comparison: vasectomy: simpler, cheaper, local anesthetic, outpatient, lower risk than tubectomy. Tubectomy: requires general/regional anesthesia, laparoscopy, higher cost, slightly higher complication risk. Yet: 2x more tubectomies than vasectomies performed globally — reflects gender dynamics in family planning decisions. WHO considers both equally safe and effective.

From Class 10

Surgical sterilization permanently prevents pregnancy by blocking the tubes through which sperm and eggs travel — vasectomy in males, tubectomy in females.
