Total Lung Capacity
Introduction
Total Lung Capacity (TLC) is the volume of air in the lungs at the end of a maximal inspiration — the sum of all four lung volumes: IRV + TV + ERV + RV.
Core concept
TLC = IRV + TV + ERV + RV = 2500 + 500 + 1100 + 1100 = 5200 mL (NCERT value). Alternatively expressed: TLC = VC + RV = 4100 + 1100 = 5200 mL. TLC cannot be measured by spirometry alone (because RV cannot be directly measured by spirometry — you cannot exhale it). Measurement requires: gas dilution techniques (helium dilution) or body plethysmography. TLC is reduced in restrictive lung disease (fibrosis, pneumonia). TLC may be normal or increased in obstructive lung disease (emphysema — air trapping increases RV).
Go deeper
Determinants of TLC: chest wall compliance, respiratory muscle strength, lung compliance. Restrictive diseases (fibrosis) reduce lung compliance → higher elastic recoil → lungs cannot expand fully → reduced TLC. Emphysema destroys alveolar walls → increased lung compliance → loss of elastic recoil → increased RV and FRC → TLC may be increased.
From Class 10
Total lung capacity (TLC) is the maximum amount of air the lungs can hold after a maximum inhalation. It is the sum of all four lung volumes.
