Molecular Diagnosis

Introduction
Molecular diagnosis uses techniques such as recombinant DNA technology, PCR and ELISA to detect diseases early, even before symptoms appear.
Core concept
Why: for effective treatment, early diagnosis and understanding of pathophysiology are very important. Conventional methods (serum and urine analysis) cannot detect very low concentrations of a pathogen (bacteria, viruses) at an early stage. PCR: very low concentrations of a bacterium or virus (when symptoms are not yet visible) can be detected by amplification of their nucleic acid; PCR is now routinely used to detect HIV in suspected AIDS patients and to detect mutations in genes of suspected cancer patients. A single-stranded DNA or RNA tagged with a radioactive molecule (probe) is allowed to hybridise to its complementary DNA in a clone of cells, followed by detection using autoradiography; the clone with the mutated gene does not appear on the photographic film, because the probe does not have complementarity with the mutated gene. ELISA is based on the principle of antigen-antibody interaction; infection can be detected by the presence of antigens (proteins, glycoproteins) or antibodies synthesised against the pathogen.
Go deeper
PCR detects nucleic acids; ELISA detects antigens or antibodies. Together they allow very early, sensitive diagnosis.
From Class 10
Modern tests can find a disease-causing germ or a faulty gene in a tiny sample of blood, long before a person feels sick.
