Natural Contraceptive Methods

NCERT Class 12 · Reproductive Health · 0 NEET questions

Natural contraceptive methods rely on the principle of avoiding the meeting of ovum and sperm. If your biological timing is off by even 24 hours, these methods fail. Biology does not offer second chances for miscalculations in the menstrual cycle.

The idea

Natural methods include periodic abstinence, coitus interruptus, and lactational amenorrhea. Periodic abstinence involves avoiding coitus from day 10 to 17 of the menstrual cycle when ovulation is expected. Coitus interruptus is the withdrawal of the penis before ejaculation. Lactational amenorrhea is based on the fact that ovulation does not occur during the period of intense lactation following parturition.

Why it matters

To prevent conception without the use of any external devices or chemical medicines.

NEET lens

NEET focuses on the 'fertile period' (day 10-17) and the specific condition of 'intense lactation' for amenorrhea. Do not confuse these with barrier methods. Remember: High failure rate, zero side effects.

Clinical significance

Lactational amenorrhea is effective only up to six months post-parturition. Failure of these methods leads to unintended pregnancy, which carries significant physiological and psychological stress. There is zero protection against STIs (Sexually Transmitted Infections) with these methods.

Introduction

Natural contraceptive methods include periodic abstinence (avoiding sex during fertile days, tracked by calendar, temperature, or cervical mucus), lactational amenorrhea (breastfeeding-induced suppression of ovulation), and withdrawal — no drugs or devices required, but lower effectiveness.

Core concept

Methods: (1) Periodic abstinence / Fertility awareness methods (FAM): Calendar method (Ogino-Knaus): avoid sex Days 8-17 (fertile window). Less reliable (variable cycles). Basal body temperature (BBT): temperature rises 0.2-0.5°C after ovulation (progesterone thermogenic). Cervical mucus method (Billings): mucus becomes clear, stretchy ('egg-white') near ovulation. Symptothermal method: combines BBT + mucus (more reliable). Effectiveness: 76-88% typical use. (2) Lactational Amenorrhoea Method (LAM): exclusive breastfeeding → prolactin → GnRH suppression → anovulation. Conditions: exclusively breastfeeding + amenorrhoea + 98% effective. Once any condition fails: other contraception needed. (3) Withdrawal (coitus interruptus): penis withdrawn before ejaculation. 78% typical use (high failure — pre-ejaculate may contain sperm, requires control). Advantages: no cost, no side effects. Disadvantages: high failure rates, require motivation + regular cycles, no STI protection.

Go deeper

LAM reliability conditions (all three must be met): (1) Baby 98% becomes unreliable. This makes LAM highly reliable in strict adherence but quickly fails when supplementary feeds introduced.

From Class 10

Natural contraceptive methods work without pills, devices, or surgery — by either avoiding sex during fertile days or by breastfeeding's natural suppression of fertility.
