Medical Termination of Pregnancy

NCERT Class 12 · Reproductive Health · 0 NEET questions

MTP is not a contraceptive method. It is a clinical intervention for pregnancy termination. Confusing the two is a common error that costs students marks in the exam.

The idea

Medical Termination of Pregnancy (MTP) refers to the intentional or voluntary termination of pregnancy before full term. It is legally and medically performed to eliminate unwanted pregnancies. It is also indicated when the continuation of pregnancy poses a risk to the health of the mother or the foetus. It is most safe during the first trimester.

Why it matters

It exists to provide a safe medical option for terminating pregnancies that are unwanted, result from rape, or pose a health risk to the mother or child.

NEET lens

NEET focuses on the legal and safety aspects of MTP. Remember: it is not a contraceptive. Questions often test the 'first trimester' safety window and the criteria for legal termination. Memorise the MTP Amendment Act implications if mentioned in your current syllabus.

Clinical significance

MTP is crucial for preventing maternal mortality due to unsafe, illegal abortions. When performed by untrained personnel, it leads to severe complications like uterine perforation, sepsis, and haemorrhage. It is a vital public health tool to manage reproductive health risks.

Introduction

Medical Termination of Pregnancy (MTP) is the deliberate ending of a pregnancy before fetal viability, legally permitted in India under the MTP Act (1971, amended 2021) up to 20 weeks (or 24 weeks in special circumstances) by a registered medical practitioner.

Core concept

India's MTP Act 1971 (amended 2021): Allows MTP up to 20 weeks by one doctor's opinion. Up to 24 weeks for: rape survivors, minors, special categories (anomaly detected, disability), with two doctors' opinion. Beyond 24 weeks: only for fetal abnormality incompatible with life (Medical Board decision). Must be performed in government hospital or certified private facility by registered medical practitioner. Reasons: contraceptive failure, risk to mother's life/health, rape, fetal abnormality. Methods: Medical MTP (<9 weeks): mifepristone (antiprogestogen blocks progesterone) + misoprostol (prostaglandin) → uterine contractions → expulsion. Surgical: vacuum aspiration (up to 12 weeks), dilatation and evacuation (12-20 weeks). Importance: safe MTP access → reduces unsafe abortions → reduces maternal mortality.

Go deeper

Mifepristone mechanism: progesterone receptor antagonist → blocks progesterone (which maintains pregnancy) → decidua breakdown + uterine contractions. Misoprostol (prostaglandin E1 analog): cervical softening + uterine contractions → expulsion. Combination 95-98% effective up to 10 weeks.

From Class 10

Medical termination of pregnancy (MTP) is the deliberate ending of a pregnancy before the fetus is viable — commonly called abortion.
